Provident Behavioral Health Breach May Have Exposed Patient Data

Mental health organization Provident Behavioral Health, located in and around St. Louis, Missouri, has reported that it suffered a data breach which may have exposed patient data. The data potentially accessed may include names, contact and demographic information, dates of birth, driver’s license or state identification numbers, Social Security numbers, medical information, and health insurance information.
